Proxy Access to MyPennMedicine

How does it work?

​

Parents of Healthy Steps patients are invited to sign up for a MyPennMedicine (MPM) portal account on behalf of their child or children (also known as a proxy account). By creating a MPM account for your child, you will be able to instantly send a message directly to your child's provider, request a refill or referral, and view test results and immunizations.

How do I gain access to a proxy account?

 

Request forms are provided below. Please note that children ages 14-17, as well as young adults over age 18, must provide a signature in person to authorize you to create a proxy account for them. After downloading and printing the correct PDF form below, you may bring your adolescent/young adult patient to our Healthy Steps office to sign the form with our staff member as a witness to their signature.

 

Onsite signature is not required if your child is 0-13 years of age.  You may use our online request form below.

Once we have received your request and any required signatures, please allow 7-10 days for the account to be activated.

We know that using MPM will save busy families time and frustration as they coordinate care for their children. If we can answer any questions, please do not hesitate to ask.

If your child is age 0-13, you may complete this form online and our team will process your request.

 

Must be completed by a parent or legal guardian of a child age 0-13.

For children ages 14-17, proxy access must be requested with a paper form signed by the adolescent in-person in our office. Please download and print the form, and bring it to our office for signature with your child.
